Analyzing 393 participants from the Alzheimer's Disease Neuroimaging Initiative, researchers tracked plasma p-tau217 levels over time using five different assay platforms. They compared cognitively unimpaired and cognitively impaired individuals with varying beta-amyloid levels. The study found that p-tau217 levels rose significantly over time, especially in those with beta-amyloid buildup. Critically, the rate of this protein's increase directly predicted the pace of cognitive decline. However, the accuracy of these predictions varied based on the specific test assay used and the patient's current cognitive stage.

Previously, diagnosing and tracking Alzheimer's required expensive brain scans or invasive spinal taps. This study shows that a simple, serial blood test measuring p-tau217 can successfully track the disease's trajectory in real-time.
