The Summary
In the SURMOUNT-MAINTAIN clinical trial, 441 adults took tirzepatide for a 60-week weight-loss period. Afterwards, 378 participants were randomized for 52 weeks to either continue their maximum tolerated dose (10 mg or 15 mg), reduce to a low 5 mg dose, or switch to a placebo. Those continuing the maximum dose maintained a 21.9% total bodyweight reduction. The 5 mg group maintained a 16.6% reduction, while the placebo group regained significant weight, keeping only a 9.9% reduction. Gastrointestinal side effects were the most common but mostly mild.
Why this is interesting
For years, many hoped weight-loss medications could be used short-term. This study proves otherwise, demonstrating that obesity is a chronic condition requiring ongoing management. If you stop tirzepatide, the weight is highly likely to return. Crucially, the trial highlights a middle ground: tapering down to a lower 5 mg maintenance dose still preserves substantial weight loss while offering a flexible, personalized strategy for those wanting to reduce their dosage. It changes how doctors and patients plan long-term health journeys.
