Keto Outperforms Other Diets for Liver Health During Weight Loss

The Summary

In a randomized clinical trial, researchers evaluated the effects of matched 10% weight loss achieved through three diets: very-low-carbohydrate ketogenic, Mediterranean, and very-low-fat plant-forward. Participants had metabolically unhealthy obesity with prediabetes and fatty liver. Although muscle insulin sensitivity improved by roughly 50% across all groups, the low-carbohydrate group achieved a two- to three-fold greater boost in hepatic insulin sensitivity. That group also experienced significantly larger reductions in liver fat, hepatic lipogenesis, glycated hemoglobin, and daily insulin and glucose levels, with no differences among groups in LDL cholesterol or apolipoprotein B.

Why this is interesting

It was long assumed that metabolic benefits stem almost entirely from shedding pounds, regardless of which foods you eat. This study challenges that view, proving that macronutrient composition directly shapes organ-specific outcomes. For individuals managing prediabetes or fatty liver disease, choosing a very-low-carbohydrate plan appears to accelerate liver fat clearance and blood sugar control far better than low-fat or Mediterranean patterns, even at identical weight loss. Crucially, these liver-specific gains occurred without raising harmful cardiovascular lipid markers.
